IBM provides two utilities for creating hibernation files:
 - hibw4win.exe which only works in Win9x or WinNT 4 (it nearly hosed my
   WinXP)
 - stndalhd.exe which is a DOS app that writes directly to a floppy.

You can find them both on the IBM website somewhere.

I used stndalhd.exe on a different machine to get a floppy image and
then it was plain sailing with mkisofs (-b allows you to specify a
floppy image to create an El Torito bootable CD) and cdrecord.
