I got my sister an account with 550 Access (at www.550access.com). I found 
a sign-up special for $4.75 a month and paid for a year in advance. They 
have local dial-up access over most of the country via agreements with 
local ISP's.

I've never had a complaint with the service... but they do not have 
telephone customer support. What do you want for $5.50?

They do not require any software, and do work with Linux, but they have 
Windows software available that will automatically configure a dial-up 
connection.
