it looks like on suse 9.1, kmilo was deactivated by default:

Yes, Kmilo is unfortunately disabled by SuSE in their rpms. To renable it, 
edit 

/opt/kde3/share/services/kded/kmilod.desktop

On line #8 in that file change it as follows:

X-KDE-Kded-autoload=true

Log out your KDE and log back in again. Thinkpad butotns should be
working with kmilo thinkpad plugin.
