You need the ipw3495 driver (kernel module) and the daemon (ipw3495d).
 You might also have to update the ieee80211 modules.  See
http://ipw3495.sf.net/ for more information.  For Ubuntu, you might
try searching synaptic for ipw3495 to see if there's an easily
installed package.

The 0.74 version of the module has worked very well for me (well, at
least since yesterday).
