It's a Lucent Agere modem.  The ltmodem package is the proper one. 
Somewhere there is a person that does make unoffical Ubuntu packages
available.  http://linmodems.technion.ac.il/packages/ltmodem/kernel-2.6/
is where I found the package (I'm using ltmodem-2.6-alk-8.tar.bz2).  I
was able to compile the models with just a configured set of kernel
headers installed (they were configured by Module Assistant when
builing the Madwifi modules for my wifi card).
