Did you do a "modprobe slamr"? AFAIK the driver consists of two parts,
the slamr module and the slmodemd daemon. Your symptoms sound like the
module is not loaded.

I'm currently in the process of trying to enable the driver under SUSE
9.0 on my HP e-pc fileserver, which has a USB connected Aztech modem. I
found that it should be supported by slmodem. The funny thing is...in
SUSE 9 there only is a slusb module, no slamr module and no slmodemd
daemon. But i didn't have time yet, to mess around, just did enable the
driver enabling in yast. (I only want to use the modem to get ring
signals to report them through the network via cidbcast).
