I am puzzled here. The trackpoint allows you to do 2 things:

 1)Raise the sensitivity of the pointing stick

 2)Enable press-to-select (if you want it) - and set the button which
this maps to.

But the middle button (btn 2) has always worked right away on all
thinkpads I have ever used (A22p,770z,T41,X22).

Are you by any chance using a thinkpad with a synaptics touchpad in it?
If so, might the synaptics configuration be what you need to tweak?

Lastly, most things seem to work with Knoppix. Try it - and if it works,
you can then compare what Knoppix does with your current distro.
