I see similar problems with my ipw3945 card talking to an apple Airport.
I've reported the bug to the ipw folks

There has been a new release since my test (1.1.0) so maybe that's been
fixed (though it's not yet marked as fixed).

I recommend doing manual association etc. when debugging instead of
trying to use another layer (network manager, wpasupplicant etc.). Just
keeps things simpler.
