I've mentioned some of this in the other thread..

You need to look at the readme - determine which drive you have - and
download the appropriate disk. All 6 disks are not necessary.

This package provides firmware for the following DVD/CD-RW drives:

  Model (Manufacturer) Diskette image filename
  SD-R2002 (TOSHIBA) FWDR1601
  DW-28E (TEAC) FWDR1601
  UJDA720 (KME/PCC) FWDR1602
  UJDA740 (KME/PCC) FWDR1603
  UJDA745 (KME/PCC) FWDR1604
  GCC-4160N (HLDS) FWDR1605
  GCC-4240N (HLDS) FWDR1605
| GCC-4240N (HLDS) FWDR1606
| GCC-4241N (HLDS) FWDR1606

For me it is 'UJDA745 (KME/PCC) FWDR1604' - which is disk-4.

My old 600E floppy adapter worked with the T40 - via the port
replicator. The alternative would be a USB floppy drive. And you'll
need a windows partition/machine as well to create this disk :)
