With one ISP - I use imap/ssl and smtp/ssl.
With work - I use ssh-portforwarding for both imap & smtp

I've tried postfix relaying over 'ssh-forwarded-smpt' port - but
monitoring postfix is a nuisance. So I let pine connect directly to
smtp relays (perhaps over ssh-forwarded ports). But since you are
using mutt - I guess you can't get arround running a local-postfix.
