I'm in Morocco right now and I'm using packet8 for my VoIP.  Its
pretty sweet, $20 a month for unlimited calls to anywhere in the US. 
The quality can go bad depending on my DSL connection, but DSL is
pretty new here and unknown by the populace.  Its basically as good as
a call from my cell was but not quite as good as my land line.

In terms of hardware, its just a little box like James described that
Primus gives out.  Doesn't require port forwarding or anything messy
on the firewall end.
