It lookes very strange. 

Can you send me an ,,lspci --v'' output and a short tutorial about
how did you setting this up? When I load snd-intel8x0m module, it
say my modem has ,,Conexant 23'' or ,,Conexant 30'' chipset, and
sl-modem-daemon say it is not supported with nor slamr nor snd-intel8x0m
module.
