This is strange, I'm using the same scripts and almost the same
configuration as you. While my sleep used to fail to resume for a
mysterious reason once in a hundred times or so, for the past few months
both sleep and hibernate have been rock solid and failed only
deterministically, for good software reasons. The big difference is that
mine is a T40.
Sorry to hear about your frustration - I know the feeling, it took me
forever to figure out the insanity that is laptop support on Linux (and
still no display hotplug!) - but I'm stumped. The only thing that comes
to mind is toggling the EnableVbetool option and related options in
/etc/hibernate/common.conf - but if it's having trouble resetting the
video, that should manifest itself on startup, not shutdown...
Might also try other kernel versions. Like a really old one - sleep has
been reliable for me since at least 2.6.17.
Finally, try Feisty (now in beta). It has all the latest and greatest
voodoo for laptop hardware support. (You could also install the relevant
package port that I'm maintaining for Gentoo,
http://overlays.gentoo.org/proj/sunrise/browser/sunrise/sys-power/acpi-support)
