i don't use ubuntu, but i use debian
and under debian s2d breaks if the nvidia module is _not_ included into
the initramfs.

so .. make sure in the file /etc/initramfs-tools/modules,  is a line like
    nvidia

and run as root 
    update-initramfs -k all -u
